Toowong Private Hospital provides treatment to people who are outside of the hospital environment. This type of community care is usually offered in place of a hospital admission, following discharge from hospital, or to help with relapse prevention. The type of care we provide is dependent upon each patient's clinical need.
Our community programs are delivered by staff working across multiple mental health disciplines. They focus on improving the health and wellbeing of all of our community patients and supporting our patients' families.
Our staff maintain regular contact with our patients and provide the very best in support and care. Our community care programs can be provided in a range of settings, such as via the telephone, at the patient's home or at another mutually convenient location.

Entry to our programs is through referral from a person's treating psychiatrist or General Practitioner. Entry is based on a person's clinical assessment, including their willingness and capacity to participate.
