REFERRAL PROCEDURE

Referral to our day patient programs can be made by any psychiatrist. There are several ways to refer your patient:

All Psychiatrists can refer to the Toowong Private Hospital Day Programs by any of the following methods:

• Completing a Day Programs Referral Form and then sending it to:

• Writing a letter of referral outlining the program you wish to refer your patient to and send it to Director – Day Programs:

• Calling the Day Treatment Services Team on:

• General Practitioners or Non- Credentialled Psychiatrists –
  If you do not have admitting privileges and have a patient you would like to admit to the community programs, please contact the Day Program Services Manager to discuss the patient’s presenting illness and determine if admission to one of the Day Patient Programs at Toowong Private Hospital is suitable, and if an admitting psychiatrist is available to take on your patient’s care.

• Credentialled psychiatrists can refer a patient to one of the Hospital’s Community Programs by contacting Toowong Private Hospital’s Community Services by either:

• Completing the Community Services Referral Form and then sending it to:

• General Practitioners or Non- Credentialled Psychiatrists –
  If you do not have admitting privileges and have a patient you would like to admit to the community programs, please contact the Community Services Manager to discuss the patient’s presenting illness and determine if admission to one of the Community Programs at Toowong Private Hospital is suitable, and if an admitting psychiatrist is available to take on your patient’s care.

If you would like to apply for clinical and admitting rights at Toowong Private Hospital, please contact the Credentialling Officer for an application pack:

Once we receive your completed application and supporting documentation, you will be invited to attend an appointment with the Director of Medical Services. Your application will then be tabled at the quarterly Medical Advisory Committee Meeting and you will be notified shortly after of the outcome. Successful applicants are credentialed for one to three years after which they are required to reapply.
